We
need a referral letter ...
This
may be written by yourself, your Doctor, social worker,
family member or friend.
The
referral needs to set out your child's problem and what
assistance you require.

NSW Isolated Patients' Travel and Accommodation Scheme (IPTAAS) |
You
may need to register with IPTAAS who pays
the bulk of travel and accommodation expenses (Convoy for
Kids Goulburn Inc. pays the balance).

| Provision
of Aids for Disabled People (P.A.D.P.) |
PADP
provides equipment either on loan, if available, or by purchasing
the necessary item required - if Government funds are available.
Physiotherapists/Occupational
therapists involved with the child will fill out all required
forms and assess the urgency of the application. |
